According to AAA guidelines, we will only accept the official AAA physical forms for athletics. These forms are mandatory for all athletes. You can get copies in the Building 4 Office or from coaches. You may also click on the following links:

If an athlete presents their physical on a different form, we will kindly reject it and guide them to use the AAA-approved version.
